Submitting s1 form at St Lukes

I've got the 2 s1 forms for my husband and I:

1. Do you need anything else apart from these forms (passport, copy of passport)?

2. My husband can't walk that far and would struggle all the way up the hill from Msida. Does he have to attend personally or can I do it for him? My recollection is that parking near St Lukes is limited (I previously have bussed and walked) . Is there any car parking?

You will certainly need your passport, and a copy thereof, though they will copy it there.

There is some parking but it is very limited and sometimes you can park in surrounding streets.

You may be able to take your husband's for him but better to check, the phone number I have is 22992345. I have always found them to be extremely helpful.
