visa overstay consequences 10 years later?

This is the following situation a friend of mine is facing.
He came to the US in 1999 on a tourist visa when he was 15, and overstay his visa / I 94 by 6 months (spent a year in a high school).
Now he has a great job offer on a cruise line, but it requires him to get visa to the many countries he will transit through, including the US.
Is there any chance he might be denied a visa or entry in the US? I told him to be candid and truthful when he gets the interview for the Visa. Any other advice ?

I don't think it's anything to worry about, the USA has much bigger immigration fish to fry.

Well, you thoughts were unfortunately incorrect ! Visa was denied for the time being.
